Patients living within the practice area of Trentham and Hanford are welcome to register at the practice. a

Please download and fill in the GMS1 Registration form below and bring to the surgery along with a some form of identification such as a recent utility bill ( in the last 3 months) and passport identification or photographic evidence.

When you register you will also be asked to fill out a medical questionnaire to provide information whilst we await your medical records from your previous practice.

You are advised to ensure that until full registration is complete that you have enough of any medications you are taking from your previous practice as we will not be able to prescribe until you have been seen by one of our GPs.

It is important that ethnicity is given so that your records are detailed in regards to ‘risk’ factors due to the COVID pandemic.  We also ask that you willingly attend a new patient registration medical check up.
