How to register
Please download and fill in the PRF1 Registration form and bring to the surgery along with a some form of identification such as a recent utility bill ( in the last 3 months) and passport identification or photographic evidence.
When you register you will also be asked to fill out a medical questionnaire to provide information and you will be invited to participate in a new patient consultation with the Health Care Support Worker.
Please note that we may not be possible to issue repeat medication on the day of registration or until such a time that we have received your full medical records from your previous general practitioner.
Please ensure that you have at least one month’s supply of your regular medication from your previous surgery.
It is important that ethnicity is given so that your records are detailed in regards to ‘risk’ factors due to the COVID pandemic.

Register Online
We are using a new online service called Register with a GP surgery that makes it easy to register at Trentham Mews Medical Centre.
New patients who are within our catchment area of Trentham and Hanford just need to fill in an online form to get started. You do not need proof of address or immigration status, ID or an NHS number.
Registration paper forms are still available at reception for people who prefer to use this method. Please ask at reception.
Non-English Speakers
